Complete the online GSRP 2021-2022 enrollment form and submit the following documentation:

• GSRP Enrollment Form

• Birth Certificate (official state copy)

• Immunization Record

• Proof of Residence (utility bill, lease, etc.)

• Verification of Income (check stub, W2, tax return, DHS statement, etc.)

• Guardianship or Custody Documents (if applicable)

• Health Insurance Information (copy of insurance card)

• Health Appraisal/Physical (due no later than 30 days after school starts)

• Additional supporting documentation (IEP, development documentation, etc.)
